Improving an elongated nasal tip with rhinoplasty
When the rhinoplasty patient wants improvement in the nasal tip contour, there is
usually the problem of excess bulk of the cartilage and soft tissue, as well as loss
of projection, and this problem affects the nose surgery technique used at our Bay
Area practice. The prognosis for tip reduction by standard recontouring of the alar
cartilages is always somewhat uncertain because of the unpredictable shrinkage of
the skin envelope in older patients. Subcutaneous defatting is helpful, particularly in
the individual with a broad appearing tip. Careful splinting is necessary, and prolonged
taped support is helpful. A cartilage graft columellar strut between the existing tip
cartilages best corrects loss of tip projection. The strut can be appropriately sutured
to achieve the desired projection of the tip unit.

Nose "Lift" - Nose Surgery
Certain older rhinoplasty patients may have so much laxity of the nasal skin that it
is impossible to effectively shorten the drooping nose with standard rhinoplasty techniques
due to limited skin shrinkage. In such instances, removal of excess skin through a transverse
incision at the root of the nose, typically in a preexisting transverse crease at the level
of the eyes, helps lift the nasal tip. The scar created by this nose surgery procedure
across the base of the bridge of the nose is inconspicuous and easily disguised with make-up
until it fades away in a few months.
